Selling a private numberplate...?

Evening all,

I've got my numberplate up for sale, little bit confused what I need, which V form etc?

Someone care to explain to me?

i know I get the V (don't know the numbers) form and fill my section in.

and then what's the next process? Someone pays for it and the transfer fees, then their £25 on top for the tax disc and admin charge etc?
