I have yes, only happened after I finished the game. I couldn't open the garage at Michael house and none of the cars were parked on the drive, Michaels car was parked on the road. Oddly enough after about 6h (real time) everything was back to normal.

From a main dealer I would imaging if you want a brand new one. As for price depends what side you need, think driver side is around £120 and passenger is £200 ish (might be other way around)

Like Colvin1991 says its probably the resistor pack that has melted, common problem. You normally find it also melts part of the wiring loom block that connects to the resistor so that will also need replacing as well if it has (wiring loom repair kit) Its not a cheap fix mind.
