    How can this happen?!

    Guessing whoever did the cam belt last didn't bother to replace the mounting bolts? That's the only explanation I can think of! They 'should' be replaced once removed but most places just re-use them. Could try some loctite on the end to stop them working loose or just buy new ones.

    Is it aftermarket or standard? I've got a Milltek on mine and it rattles sometimes at idle. Probably just a bit of loose heat shield, I know some people have replaced the mounts which can help. But you won't really know where the issue is until you get under it and have a poke round!
